Refills:  For your convenience, it is recommended that you contact your pharmacy and ask them to fax a refill request to our office at: (336) 275-3550. We will authorize the refill request within 48 hours by contacting your pharmacy directly. Prescriptions are ONLY refilled by electronic filing to your pharmacy from our office. E-mail us at prescriptions@feminawomenscenter.net to let us know that you are requesting a refill on your prescription. We believe in a green, earth friendly environment and have all prescription requests sent electronically.

Billing: Primary insurance will be filed as a courtesy by the practice.  Any secondary insurance is to be filed by the patient. Copies of the Explanation Of Benefits and claims will be provided by the practice upon request. We will gladly fill out FMLA or disability forms. However, we require a fee of $10.00 that must be paid in advance to fill out the paperwork. We will gladly send copies of your medical records to other physicians at no cost to you. However, should you request copies for yourself or for other sources, a $10.00 fee will apply.  If you have any questions please feel free to e-mail us at: billing@feminawomenscenter.net.

I understand that I am financially responsible for, agree to pay and guarantee payment in full of any charges for services provided to me by The Femina Women’s Center; even if such treatment is not covered by insurance.

Special Needs: When you call our office for an appointment, (336) 389-9898, please specify whether you absolutely need a female doctor or require any special need such as a hearing-impaired interpreter or a Spanish-speaking interpreter.
