Health Insurance
| | | We accept most of the major medical insurance companies and we are accepting new patients daily.
Some of the insurance carriers we accept are:
Blue Cross-Blue Shield/ Cigna/ United Health Care/ MedCost/ Well Path/ Aetna
Medicaid and private health insurance may cover the cost of the ParaGard® or Mirena® IUD. Always check with your own health insurance provider to see what your out-of-pocket costs may be. It is our patients’ responsibility to check with their own health insurance companies on any type of procedure done with our OBGYN office.
Payment plan options and requirements are as follows:
- You will pay 100% of the patient responsibility on the first appointment.
- Other payment schedules are available to you as long as you contact the Femina billing office and are approved for a payment plan.
- There will be a $25.00 fee for checks returned for insufficient funds.
- Co-pays must be paid at time of appointment.
- You are responsible for notifying your insurance company of an expected hospital stay. In most cases pre-certification is required.
In the event your account becomes delinquent for a period of thirty (30) days, you hereby acknowledge that you will be immediately responsible for the balance, interest, court costs and/or attorneys’ fees that result due to an outstanding unpaid balance.

To ensure that money is available to pay for the healthcare provided by our office, each individual insurance plan may vary according to the agreement made. Whoever operates your health care plan can explain in better detail what it covers and tell you about your out-of-pocket co-pays and costs. It is ultimately the patient’s responsibility. While you are here at our office you will need to sign a HIPPA form as well as any insurance papers. To reach our goal in getting you in and out of our office in one hour, please fill out our patient consent , disclosure of medical information, and our authorization to leave phone messages forms and bring them with you to your scheduled appointment.
