| | | Femina Women’s Center is accepting new patients and strives to get you in and out of our office in one hour. We ask that you fill out our online patient forms. (hotlink to the form within the site.) New patients should arrive fifteen minutes prior to the scheduled time to register. You may be asked to reschedule your appointment if you are more than fifteen minutes late.
Appointments If you need to cancel your appointment, please do so at least 24 hours prior to your scheduled visit by calling (336) 389-9898. We reserve the right to request all no shows to pay a fee for a scheduled visit missed. Forms that you will need to fill out and leave with us at the time of your appointment are, disclosure of medical information, patient consent form, and the authorization to leave phone messages.
Cell phones are not allowed in our offices. Please do not use them during your office visits so that we may provide you with our complete attention to serve your needs. This policy is also in place to maintain our goal to have you in our office for only an hour. Some frequencies of cell phones interfere with our monitors for monitoring your baby’s heart rate.
While you are in our waiting area, out of respect for all our patients’, kindly refrain from eating or drinking. Many of those we see may be unable to eat or drink due to testing.
Updates Kindly make us aware of changes in contact information or insurance coverage. By updating your phone number we will be able to contact you in a timely manner with lab work, test results or changes to your appointment time in case our doctors are called for an emergency.
Lab Results We will review lab work, radiology reports, biopsy results, etc. at a subsequent visit. You will receive well-woman exam visit results via mail or phone call. Pap smear results will be mailed to you if they come back normal. When an abnormal lab result from your pap smear is shown, our nurse will call to schedule a subsequent office visit to discuss your results.
Co-Pays/Claims You are expected to pay for office services at the time of your visit including any co-payment required by your insurance provider. If you come to your appointment and do not have your required co-payment, we reserve the right to reschedule your appointment for a time when you will have your proper co-payment. At Femina Women’s Center we participate with most major HMOs, government plans and insurance companies’ health coverage. If you are unsure about your coverage, please contact your insurance company prior to your visit with us. It is our patients’ responsibility to check with their own health insurance providers on any type of procedure performed in our OBGYN office. Our office will file insurance claims for obstetric and surgical care and will assist you in filing for regular office visits, laboratory work and/or X-rays.
Prescriptions Prescriptions are ONLY refilled by electronic filing to your pharmacy from our office. We believe in a green, earth friendly environment and have all prescriptions sent electronically. If you can’t reach us during normal office hours it is best to call your pharmacy and ask them to fax a refill request to our office at (336) 275-3550. We will authorize the refill request within 48 hours by contacting your pharmacy directly.
Payment of fees To reflect the quality of care you will receive we have certain fees in place. Payment is expected at the time of your visit. We accept cash, checks, money orders, MasterCard and Visa. We will gladly complete various insurance forms, or disability forms; however, please allow at least five business days for completion of these forms. A fee for our office to fill out various insurance or disability forms will be processed. An additional fee will also be charged for any returned checks.
After Office Hours For urgent calls after normal office hours please call our office and you will be connected to the doctor on call. Femina will respond to inquires promptly. Compliance with this policy may avoid hospital visits. In case of a real emergency dial 9*1*1. Our office number is (336) 389-9898.
Children at Time of Appointment So that we may provide you with our complete attention to serve your needs and to maintain our goal to have you in our office for only an hour, children under the age of two months are not allowed in the building. Please make other day care arrangements for your child. Failure to comply with our policy will result in your appointment being rescheduled for a time when you will have childcare available.
